CASE MANAGER

Site: RSBA HQ
Reports to: Lead Case Manager

THE ORGANIZATION:

  • Rising Star Business Academy (RSBA) is a 501c3 youth leadership development organization, whose purpose is to offer an integrated human development approach to high quality workforce and leadership development, education, resources, and services. Through the creation of an inclusive, equitable and nurturing environment, with the provision of culturally relevant and rigorous instruction. The goal is to improve the overall financial stability and sustainability of the organization.​

THE PROGRAMS:

  • Moreno Valley YouthBuild​

  • California Violence Invention and Prevention

THE POSITION:

  • Assessing clients through assessments and creating plans through implementation, coordinating, mentoring, and evaluating options of services required to meet each client’s health and human service needs. Providing services afterschool schedule and, on weekends to accommodate schedule, and being able to provide services on a hybrid platform when needed.

ROLES AND RESPONSIBILITIES:

  • Provide goal setting and achievement through service plans

  • Develop individual service plan with participants and families

  • Work with organization vendors to provide resources to participants and families

  • Complete intakes and assessments

  • Working with local providers to ensure continuum of care

  • Meet with participants weekly

  • Make referrals to appropriate providers

  • Work some weekends (required)

  • Attend weekly case review meetings

  • Data entry (monitoring participant progress)

  • Attend professional development trainings and community events(as needed)

  • Meet weekly with Lead Case Manager

JOB QUALIFICATIONS:

  • Driver’s license and reliable transportation

  • Strong Organizational Skills

  • Bachelor’s Degree in psychology or related field or 2 years’ experience case manager with atrisk youth

  • Compassionate and able to relate to different participants with various needs

  • Motivational to encourage participants to follow their care plans

  • Strong verbal and written communication skills to explain to clients and maintain good case records

  • Critical thinking and problem solving to determine the best care plan for each participant after assessing participants, analyzing notes from health care and social workers

  • Computer literacy (proficiency in Microsoft suite)
